Summary	returnError returns JSON without header
Description	In us_helpers.php: returnError exits with json_encode. This returns Javascript with a text header, not a Javascript header. Solution: Add header('Content-Type: application/json;'); at line 628 to return JSON with an appropriate header.

I've never had this cause an issue, but I'm happy to add this. Thanks for the ticket!

It effects the unofficial TFA plugin. The plugin uses a .ajax call to validate that TFA is correctly setup and interprets the result. This requires the header to be set, or the browser will not parse the returned JSON correctly.
