<p>I cannot duplicate this on windows 10. Are you using XAMPP? The only thing I can think of is that maybe your db user doesn't have the permissions to create a new table. The way this works is when you create a form called test it creates two tables. test and test_form. Test is your data and test_form is your form parameters.  Can you try creating a new form and see if that is happening?</p> <p> </p>

Issue was not followed up on and has no updates, cannot be reproduced. Please report the issue again if it remains.
